Page Avenue, no traffic signs were  posted at the advanced warning area, transition area, or the activity area of the two-day long work zone where employees were replacing pipe in an excavation.  The dump truck and trackhoe were both located in the inner-most westbound lane.

29 CFR 1926.200(g)(2):  All traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA:  a/  At the job site on or about 06/11/14, a sufficient number of traffic cones and/or barriers were not were not posted at the activity area along on-coming lane to guide motorists away from the parked dump truck and trackhoe.  The work zone existed along the westbound lane of E. Page Avenue.  Employees were replacing pipe in an excavation.  Heavy traffic was observed driving past this job site while the work was progressing.

The trench measured approximately 35"W X 55"D X 89"L.  Neither a ladder, ramp, nor other safe means of egress was provided.

Adequate precautions were not taken to protect employees against the hazards posed by water accumulation:  a/  At the job site, one employee used a hack saw to cut through a water pipe located deep inside of the trench.  Water was pouring from the pipe onto the floor of the trench which was no longer visible.  Adequate precautions such as a special support or shield system, continuous water removal through the use of a pump, or the use of a safety harness and a lifeline were not in use.
