29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. The employer did not have in writing evidence that such a survey had been performed:  a/  At the job site, an engineering survey had not been performed in writing by a competent person prior to the demolition of a multi-story, residential home.

Employees were authorized to approach the unsupported north and west walls of this structure to retrieve bricks.

29 CFR 1926.859(g): During demolition, continuing inspections by a competent person were not made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. Employee(s) were permitted to work where such hazards existed until they were corrected by shoring, bracing, or other effective means:    At the job site, a competent person was not present when demolition operations occurred on a multi-story, masonry home. The following hazards/violations were noted:    a/  Lack of protective head gear.    b/  Lack of safety glasses.    c/  Lack of an engineering survey.    d/  Permitting employees to retrieve bricks from the base of the unsupported, north and west masonry walls.
