29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.      On or about June 11, 2014 at residential structure located at or near 14101 Overcreek, Woodlands Edge, AR 72211, employees were applying stucco to residential structure under construction. The employer did not ensure employees were protected by the use of a fall protection systems while applying stucco on exterior walls of residential structure from scaffolding. Employees were observed/exposed to trip and elevated fall hazards of greater than 10 feet to unfinished ground below.

29 CFR 1926.452(a)(2): Crossbracing was not installed between the inner and outer sets of poles on double pole scaffolds:       On or about June 11, 2014 at residential structure located at or near 14101 Overcreek, Woodlands Edge, AR 72211, employees were applying stucco to residential structure under construction. The employer did not ensure crossbracing was installed on both sides of scaffold. Employees were observed/exposed to elevated fall hazards of greater than 10 feet to unfinished ground below.
