29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Jose Aparicio - 1315 Cromwell Street, Wyomissing, PA - The employer did not ensure the use of fall protection for the employee working on the roof who were exposed to a nine (9) feet ten (10) inch fall, on or about June 9, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).    (a) Jose Aparicio - 1126 Sheridan Street, Wyomissing, PA - The employee was using a 30 foot metal extension ladder in the collapsed position to work on the drip edge at nine (9) feet ten (10) inches high - which was approximately seventy five (75) percent of the working length of the ladder, on or about June 4, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
