29 CFR 1910.157(c)(1):  The employer did not mount, locate and identify portable fire extinguisher so that they are readily accessible to employees without subjecting the employees to possible injury:  a)  In the northeast corner of the warehouse.  On or about June 11, 2014, and at times prior thereto, in case of an emergency, employees were exposed to burns from fire and explosion, as the Badger portable fire extinguishers were sitting on the floor and not identified with proper signage.   b)  In the southwest corner of the storage shed.  On or about June 11, 2014, and at times prior thereto, in case of an emergency, employees were exposed to burns from fire and explosion, as the Buckeye portable fire extinguisher was sitting on the floor and not identified with proper signage.   c)  In the center of the west wall of the storage shed.  On or about June 11, 2014, and at times prior thereto, in case of an emergency, employees were exposed to burns from fire and explosion, as the Buckeye portable fire extinguisher was sitting on the floor and not identified with proper signage.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinder is designed to accept a cap, were not in place, hand-tight, except when cylinders are in use or connected for use:   In the storage shed, on or about June 11, 2014, and at times prior thereto, employees were exposed to burns and injuries from fire and explosion, as all cylinders of oxygen and acetylene were not capped when not in use.

29 CFR 1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  In the storage shed, on or about June 11, 2014, and at times prior thereto, employees were exposed to burns from fire or explosion, as cylinders of oxygen and acetylene were stored directly adjacent to each other, where they moved supplies and equipment.

29 CFR 1904.29(b)(1):   A log of all recordable, work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  a)  On or about June 11, 2014, at the inspection site, an incomplete description recorded in column (F) of the OSHA Form 300 Log of Work-Related Injuries and Illnesses for CY 2014 materially impaired understandability of the nature of the  injury that occurred on 06/02/2014, Case No. 2, failing to describe the object/substance that caused the injury.
