29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: On or about June 11, 2014 employees were exposed to electrical hazards as the employer failed to examine power electrical cord for missing ground prong.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used. On or about June 11, 2014 employees were exposed to two instances of electrical hazards. 1. Electrical power cord being used to supply electricity to tools has black electrical tape wrapped around the sheathing to temporary fix tears. 2. Electrical power cord to the circular saw has duct tape to repair torn cord sheathing.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 11, 2014, employees were exposed to fall hazards in excess of six feet.
