29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together but the scaffold's structural integrity was not maintained by the user:    a) Jobsite, 500 Treadway Lane, Creighton, PA:  On or about June 16, 2014, employees erected a ladder jack scaffold using ladders manufactured by different manufacturers that had different maximum load capacities and were made of different materials with the scaffold platforms and brackets manufactured by Werner, and the scaffold failed.

29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less:     a) Jobsite, 500 Treadway Lane, Creighton, PA, Werner Aluminum platform:  On or about June 16, 2014, 4 employees were working on a ladder jack scaffold and overloaded the weight capacity of the Werner Task Master Aluminum plank, model # 2024, that was rated at 250 pounds each and one person per platform, by having 3 employees with tools (at least 520 pounds) on the far right platform and 2 employees with tools (at least 330 pounds) on the middle platform.    b) Jobsite, 500 Treadway Lane, Creighton, PA, Werner scaffold brackets:  On or about June 16, 2014, 4 employees were working on a ladder jack scaffold and overloaded the weight capacity of the Werner brackets, model # AC 10-20-02, that were duty rated 300 pounds each, by having 3 employees with tools, working on the same platform that was supported by the brackets.  The bracket on the ladder that failed also was supporting another platform that had 2 employees working on it.    c) Jobsite, 500 Treadway Lane, Creighton, PA, Keller ladders:  On or about June 16, 2014, 4 employees were working on a ladder jack scaffold and overloaded the weight capacity of 2 Keller fiberglass ladders, model # 5024, used to support the platforms that were rated at 250 pounds each, by having 2 employees, with tools, working on the platform between the ladders, that was also supporting 2 other platforms with 3 employees working on the far end one.    d) Jobsite, 500 Treadway Lane, Creighton, PA, Caprum ladder:  On or about June 16, 2014, 4 employees were working on a ladder jack scaffold and overloaded the weight capacity of the Caprum Household ladder, model #385-24, that was rated at 200 pounds by having a scaffold bracket attached to it with a minimum usage rating of 300 pounds and a scaffold platform on it with a minimum usage rating of 250 pounds.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    a) Jobsite, 500 Treadway Lane, Creighton, PA:  On or about June 16, 2014, 4 employees were exposed to a fall hazard when working on a ladder jack scaffold at approximately 16 feet above the ground, without fall protection.  The employees were installing wood along the edge of the roof when the scaffold collapsed and the employees fell to the ground.

29 CFR 1926.451(g)(2): The employer did not provide fall protection for employees erecting or dismantling supported scaffolds:  a) Jobsite, 500 Treadway Lane, Creighton, PA:  On or about June 16, 2014, four employees erected a ladder jack scaffold to work along the side of a commercial building, without fall protection.  The employees were going to be replacing the existing roof on the commercial roof and had set up the scaffolding to initially work from it.  The roof was a barrel roof and was available to use to attach fall protection to.  The fall hazard to the employees was 16 feet to the ground.

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another:    a) Jobsite, 500 Treadway Lane, Creighton, PA:  On or about June 16, 2014,  Four employees were working on ladder jack scaffold that was erected with 3 platforms that were bridged to one another at 2 areas in the middle.  The scaffold was approximately 16 feet above the ground and with each platform being 24 feet long by 12 inches wide, was a total of 70 feet in length.

Each ladder supported 2 platforms; one had 2 employees working on it and the other had 3 workers on it.    b) Jobsite, 500 Treadway Lane, Creighton, PA, Caprum aluminum ladder:  On or about June 16, 2014, 4 employees were working on a ladder jack scaffold and overloaded the weight capacity of the Caprum household ladder, model #385-24, that was rated at 200 pounds by having a scaffold brackets attached to it with a minimum usage rating of 300 pounds and a scaffold platform on it with a minimum rating of 250 pounds.
