29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about 6/11/2014, on the 2nd floor of Morgan City High School, the employer allowed employee(s) to use the Gruvlok pipe threading machine which was plugged into a flexible power cord that did not have any strain relief on the female end of the cord.
