29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a.  On or about June 17, 2014 - at the above addressed jobsite, floor hole openings used for ladder access were not adequately guarded throughout the building, thereby exposing employees and subcontractor employees to fall hazards of approximately nine (9) feet.

29 CFR 1926.502(b)(6): Guardrail systems shall be so surfaced as to prevent injury to an employee from punctures or lacerations, and to prevent snagging of clothing.    a.  On or about June 17, 2014 - at the above addressed jobsite, the wood (2x4) guardrail systems throughout the jobsite were constructed using double headed nails which did not prevent snagging of clothing, and/or prevent injury to an employee from punctures or lacerations.
