The most recent example of this was found on the job site located at 18962 Pratt St., Elkhorn, NE 68022 where the employer has unprotected employees roofing a new residential home without the use of fall protection.

29 CFR 1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:  The employer is failing to protect employees from potential fall hazards associated with using ladders as a means of access and egress from elevated work surfaces where the ground to eave is greater than 6 feet and the roof pitch is more than 4:12.  The most recent example of this was found on the job site located at 18962 Pratt St., Elkhorn, NE 68022 where the employer has employees using an aluminum ladder as a means of access to the roof of a new residential home without extending the ladder a minimum of three feet over the upper landing surface.
