29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person:  On or about June 17, 2014, and at times prior thereto, a JLG platform supported by a Terex Telehandler was used to gain access to and from a roof.  A qualified person did not ensure the platform was loaded in accordance with the design of the platform, exposing employees to the hazards of falls.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  On or about June 17, 2014, and at times prior thereto, a JLG platform supported by a Terex Telehandler was used to elevate personnel to gain access to and from a roof.  The platform was not secured and attached to the forks, exposing employees to the hazards of falls.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about June 17, 2014, and at times prior thereto, employees did not use fall protection when using a JLG platform supported by a Terex Telehandler to gain access to and from a roof, exposing employees to the hazards of falls greater than 10 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about June 17, 2014, and at times prior thereto, employees performing decking operations were working near a leading edge in commercial construction without the use of fall protection equipment, exposing employees to the hazards of falls of 6 feet or more to the ground.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about June 17, 2014, and at times prior thereto, employees were accessing a roof of a commercial building under construction with a Werner ladder that did not extend at least 3 feet above the landing, exposing employees to a fall hazard greater than feet to the ground below.
