29 CFR 1910.212(a)(1):  One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  South wall of Panel Shop. Ridgid Drill Press, Model DP15000, Serial 02082E0070.  On or about June 17, 2014, and at times prior thereto, employee(s) were exposed to an amputation hazard from the lack of a guard at the point of operation, while drilling 1-inch holes into 8-inch round metal plates.

29 CFR 1910.215(a)(4):  Work rest was not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage:  South wall of Panel Shop, Dayton 6-inch Bench Grinder, Model No. 2Z425U, Lot No. 3607, 1/4 HP, 3450 rpm, 3 1/2 amps.  On or about June 17, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, as the work rest was not adjusted to a distance of 1/8-inch or less to the wheel, as they deburred metal parts.

On or about June 17, 2014, and at times prior thereto, employees were exposed to a struck-by hazard, as there was no adjustable tongue guard at the top, and the peripheral member was at a distance of greater than 1/4 inch from the wheel, as they deburred metal parts.

On or about June 17, 2014, and at times prior thereto, employees were exposed to a struck-by hazard from movement of the grinder not being secured with all bolts onto the pedestal, as they deburred metal parts.
