29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s):    (a) On June 18, 2014, at the facility located at 501 N National Ave, Springfield, MO., The employer did not ensure that Tailift Powered Industrial Truck used to move equipment and materials in the yard area, was taken out service or repaired for the following defects:      (1) The Emergency Brake was in operative and could not keep the unit from travelling in forward and reverse gears.    (2) The two Foot Pedal Controls did not have any Non-Slip Resistant covers exposing bare metal.    Abatement certification received.

29 CFR 1910.179(b)(5): Rated load marking. The rated load of the crane shall be plainly marked on each side of the crane, and if the crane has more than one hoisting unit, each hoist shall have its rated load marked on it or its load block and this marking shall be clearly legible from the ground or floor.            (a) On June 18, 2014, at the facility located at 501 N National Ave, Springfield, MO., The employer did not ensure that the rated load capacity of the hoist equipment and the overhead bridge were plainly marked and visible to indicate the rated load.               Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
