29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems or personal fall arrest system:       (a)  122 South Walnut Street, Front Porch:  Employees were working on a flat roof that was 10 feet 10 inches above ground level while preparing to install insulation behind house siding.  Employees were not protected from falling from the roof to the ground below by a guardrail system or personal fall arrest system, as observed on June 18, 2014.       Abatement Certification Required for this Item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  122 South Walnut Street, Front Porch:  Employees used a 16 foot Werner extension ladder to access the front porch roof.  The ladder was not extended at least 3 feet above the landing.  The Werner extension ladder was extended 8 inches above the porch roof landing, as observed on June 18, 2014.       Abatement Certification Required for this Item.
