29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 06/19/14 in the shop area; employer failed to develop a written procedure describing control of the two energy sources ( air and electric ) for use by employees performing service and maintenance work, such as but not limited to, changing the saw blade, cleaning in and around the saw blade, on the Marvel 8 Mark III vertical band saw to protect against being cut by the unexpected movement of the saw blade.      b) On or about 06/19/14 in the shop area; employer failed to develop a procedure for use by employees performing service and maintenance work, such as but not limited to, changing the saw blade, cleaning in and around the saw blade, on the DoAll Model C-305A horizontal band saw to protect against being cut by the unexpected movement of the saw blade.      ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    a) On or about 06/19/14 in the shop area; employer failed to train employees involved in performing service and maintenance work, such as but not limited to, changing the saw blade, cleaning in and around the saw blade, on the Marvel 8 Mark III vertical band saw the proper method and means for control of the two energy sources ( air and electric ) to protect against being cut by the unexpected movement of the saw blade.      b) On or about 06/19/14 in the shop area; employer failed to train employees involved in performing service and maintenance work, such as but not limited to, changing the saw blade, cleaning in and around the saw blade, on Doall Model C-305A the safe application of a lock on the energy isolation device when capable.      ABATEMENT CERTIFICATION REQUIRED
