This condition exposed employees to impalement hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Elementary School, East Side: On or about June 12, 2014 and at times prior thereto, employees directed to remove formwork with sledge hammers were not using safety glasses. This condition exposed employees to eye injuries from flying debris. (b) Elementary School, East Side Foyer: On or about June 12, 2014 and at times prior thereto, employee directed to drill holes into metal door frames to install door hardware was using safety glasses. This condition exposed the employee to eye injuries from flying debris.

This condition exposed employees to electrical shock and burn hazards from contact with the energized circuits. 29 CFR 1926.403(b)(1)(ii): Safety of equipment was not determined on the basis of mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: (a) Elementary School, Electrical Room: On or about June 12, 2014 and at times prior thereto, a combustible, cardboard cut-out was covering the live 480 volt electrical distribution panel. This condition exposed employees to electrical shock and burn hazards. (b) Elementary School, Electrical Room: On or about June 12, 2014 and at times prior thereto, a combustible, cardboard cut-out was covering the live 480 volt electrical distribution panel. This condition exposed employees to electrical shock and burn hazards. (c) Elementary School, Electrical Room: On or about June 12, 2014 and at times prior thereto, a combustible, cardboard cut-out was covering the live 240 volt electrical distribution panel.
