29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through: a) Production Area: The employer failed to cover/fill in holes by the extruders in the Production Area, which measured 3 inches wide, exposing walking employees and powered industrial truck operators to tripping hazards. b) Production Area/Dock door leading to back of the facility : The employer failed to cover/fill in gap leading from the Production Area to the exit door, which measured 2 inches wide, exposing walking employees to tripping hazards. c) Production Area: The employer failed to cover/fill in holes in the area of Press #2 on the Production floor, which measured 3 inches wide, exposing the employees and powered industrial truck operators to tripping hazards.

29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating: a) Production Area: The employer failed to ensure that a LP gas cylinder was secured on the Hyundai #25C-5E powered industrial truck. The latch designed to secure the cylinder was broken. The PIT operator was exposed to a struck by hazard while operating the industrial truck to move automotive parts in and around the process area.
