29 CFR 1910.95(c)(1): 29 CFR 1910.95(c)(1):  A continuing, effective Hearing Conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8-hour Time-Weighted Average sound level (TWA) of 85 decibels (dBA).    The following employees were exposed to noise levels at or above the 8- hour TWA of 85 dBA while operating Headers and Threaders and the employer did not ensure a Hearing Conservation Program had been instituted.     a) On or about 6/27/14, a Threader Operator within the Threading Department was exposed to an 8-hour TWA noise level of 89.48 dBA with a dose of 93.1% during a 259 minute sampling period allowing 221 minutes for zero exposure.     b) On or about 6/27/14, a Threader Operator within the Threading Department was exposed to an 8-hour TWA noise level of 89.60 dBA with a dose of 94.6% during a 260 minute sampling period allowing 220 minutes for zero exposure.     c) On or about 6/27/14, a Header Operator within the Heading Department was exposed to an 8-hour TWA noise level of 88.07 dBA with a dose of 76.5.% during a 269 minute sampling period allowing 211 minutes for zero exposure.     d) On or about 6/27/14, a Header Operator within the Heading Department was exposed to an 8-hour TWA noise level of 89.25 dBA with a dose of 90.1% during a 255 minute sampling period allowing 225 minutes for zero exposure.       There is no abatement certification or documentation required for this item.

The following employees were exposed to noise levels at or above the 8- hour TWA of 85 dBA while operating Headers and Threaders and the employer did not ensure a monitoring program was developed and implemented.   a) On or about 6/27/14, a Threader Operator within the Threading Department was exposed to an 8-hour TWA noise level of 89.48 dBA with a dose of 93.1% during a 259 minute sampling period allowing 221 minutes for zero exposure.   b) On or about 6/27/14, a Threader Operator within the Threading Department was exposed to an 8-hour TWA noise level of 89.60 dBA with a dose of 94.6% during a 260 minute sampling period allowing 220 minutes for zero exposure.   c) On or about 6/27/14, a Header Operator within the Heading Department was exposed to an 8-hour TWA noise level of 88.07 dBA with a dose of 76.5.% during a 269 minute sampling period allowing 211 minutes for zero exposure.   d) On or about 6/27/14, a Header Operator within the Heading Department was exposed to an 8-hour TWA noise level of 89.25 dBA with a dose of 90.1% during a 255 minute sampling period allowing 225 minutes for zero exposure.
