29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about June 19, 2014 and times prior thereto, employees of Carlos Coronado Masonry were observed bricklaying on the outside of a two story residence being constructed at or near 5500 Slimbridge in Jonesboro, Arkansas.  An employee was observed working below another employee without the use of a protective helmet, exposing employees to overhead hazards.

Employees were observed working from a two-tier scaffold which had not been designed by a qualified person.

The employees were observed walking and working from scaffold platforms 6 inches wide, exposing employees to fall hazards of greater than 10 feet to the ground level below.

The employees had performed operations with the use of a two-tier scaffold.  Platforms used on the scaffolding were at heights greater than 10 feet with no means of protection from falling to the lower level, exposing employees to fall hazards.

Crossbraces were used as a means of access.    On or about June 19, 2014 and times prior thereto, employees of Carlos Coronado Masonry performed bricklaying outside of a two story residence being constructed at or near 5500 Slimbridge in Jonesboro, Arkansas.  Employees were observed accessing a scaffold platform at a height of 14 feet by means of climbing crossbracing and frame members, exposing employees to fall hazards.

An employee was observed working from the steep sloped rooftop of the residence to perform bricklaying without the use of fall protection, exposing the employee to fall hazards of greater than 14 feet.
