29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof Front of House - Employees installing roof shingles were exposed to a fall hazard from approximately nine (9) feet - sixteen (16) feet without the use of a personal fall protection system, guardrail system or safety nets, on or about June 20th, 2014.  b) Roof Back of House - Employees installing roof shingles were exposed to a fall hazard from approximately nine (9) feet - sixteen (16) feet without the use of a personal fall protection system, guardrail system or safety nets, on or about June 20th, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) Front of Building - The portable extension ladder used to access the roof did not extend three (3) feet above the upper working surface exposing employees to a fall hazard during access and/or egress, on or about June 20th, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
