29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:    (a) Cutting bench on front side, new residential construction located at 3040 Haven Mill Lane, Milton, GA 30004: The employer was exposed to amputation hazards while operating Makita ( Model - 5007F) 7 1/4-inch Circular Saws with a wood shim between the lower and upper guards which prevented the lower guard from retracting after a cut was made.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Upper roof, new residential construction located at 3040 Haven Mill Lane, Milton, GA 30004:Two employees were exposed to falls of approximately 22-feet to the lower level while installing Hardie-Shake Siding from a 6/12 pitched roof with no fall protection system in place.    (b) Lower roof, new residential construction located at 3040 haven Mill Lane, Milton, GA 30004: One employee was exposed to falls of approximately 14-feet, 7-inches to the lower level while installing Hardie-Shake Siding from a 4/12 pitched roof with no fall protection system in place.
