29 CFR 1926.601(b)(14): All defects were not corrected before motor vehicle(s) were placed in service: 12 Highland Commons West, Berlin, Massachusetts; Caterpillar 416C, SN 1WR10159 was placed into service with uncorrected defects: a) The window latches were broken and did not prevent the rear window from closing onto the controls while the machine was in use. b) The amber warning light on the back of the cab was broken at the time of the accident and still not repaired as of August 28, 2014.
