29 CFR 1910.101(a): The employer did not determine that compressed gas cylinders under his control are in a safe condition to the extent that this can be determined by visual inspection or other inspections in accordance with Compressed Gas Association Pamphlets C-6-1968 and C-8-1962, which is incorporated by reference as specified in Sec. 1910.6:    a). On or about 6/23/14, on site, for employees that have potential exposure to nitrous oxide from operating/administering the scavenging unit used for anesthetic uses during dental work. The employer did not conduct any type of inspection of the scavenging unit comprised of compressed gas cylinders of nitrous oxide. The scavenging unit was taped in various areas to prevent leaking.

29 CFR 1910.101(b): Section 3.2.1, Compressed Gas Association Pamphlet P-1-1965, as adopted by 29 CFR 1910.101(b):  Where removable caps are provided for valve protection, such caps were not kept on cylinders at all times except when cylinders were in use.  a). On or about 6/23/14, in the basement on site, for employees instructed to handle nitrous oxide compressed gases when in use. The employer stored cylinders, in the same vicinity, that were not in use without their caps.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a). On or about 6/23/14, on site, for employees handling corrosive chemicals such as Potassium Hydroxide when adding X-Ray Developer for developing dental films. The employer did not have an appropriate eye wash station on site.

On or about  on 6/23/14, site, for employees with potential exposure to bloodborne pathogens when handling dental instruments and assisting the dentist with patients. The employer did not provide a copy of the exposure control plan to the employees.
