29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:   On or about June 23, 2014, at the above addressed site, an employee trimming palm trees was exposed to a fall hazard of approximately 35 feet when working from the basket of an aerial lift and not wearing a body belt or harness with a lanyard attached to the boom or basket of that lift.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  On or about June 23, 2014, at the above addressed site, an employee trimming palm trees over and adjacent to his head was exposed to struck by head injuries while not wearing head protection equipment.
