29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.     a) Cutting Department - On or about June 19, 2014, the metal clips that were on the floor at Komax Cutter A-1 created a tripping hazard.  The metal clips were on a spool and looped on the floor from the spool to the Komax cutter.

Maintenance - The Dayton Commercial grinder tongue guard was not adjusted to one-fourth inch from the wheel.  The Baldor grinder-buffer did not have a tongue guard.                     2. Engineering Technician work table - The Black and Decker 6 inch grinder did not have a tongue guard.
