29 CFR 1910.303(g)(1)(i)(C): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of 29 CFR 1910.303:   a) On June 23, 2014, in the Engineering Office,  a table was blocking the workspace in front of a 120 volt electrical panel.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a) On June 23, at the facility, employees using chemicals such as, but not limited to, sodium hydroxide were not provided adequate training about the new label elements and safety data sheet format.
