OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that no form of auxiliary escape was provided from the derrickman's platform, exposing employees to burn and fall hazards.    On and before June 23, 2014, the employer did not provide an auxiliary escape means for employees working from the derrickman's platform, exposing them to burn and fall hazards in the event of a flash fire.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    On June 23, 2014, employees were exposed to a fall hazard on the drill rig platform. The V gate opening for transferring pipe sections up onto the work platform was open approximately 8 feet wide with a fall distance of approximately 10 feet above the ground while employees were working and standing near the open sided edge.

29 CFR 1910.181(i)(3)(v): Hoisting, lowering, or swinging was done while a person was on the load or hook:    On and before June 23, 2014, an employee stood on and rode the elevator section (under the hook device) that was attached and suspended from the derrick hoist block. The employee rode the elevator while the drilling operator activated the hoist block and elevator up to the derrickman's platform (monkey board) for the employee to perform work such as but not limited to mast lighting installation and pipe racking.
