29 CFR 1926.501(b)(12): 29 CFR 1926.501(b)(12) Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.   (a) Jobsite, 2805 Freeport Road, Harmar Township, PA:  On or about 06/18/14 employees working on the mezzanine section of the building engaged in precast concrete operations were not protected from falling approximately 20 feet to the ground below.  The employees were on the mezzanine section scraping and sweeping and were not protected from falling by guardrail systems, safety nets or personal fall arrest systems.

29 CFR 1926.503(c)(3):The employer failed to conduct retraining when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. Note: The following appendices to subpart M of this part serve as non-mandatory guidelines to assist employers in complying with the appropriate requirements of subpart M of this part:   (a) Jobsite, 2805 Freeport Road, Harmar Township, PA:  On or about 06/18/14 retraining on fall protection was not conducted for employees who were working at heights up to 20 feet without adequate fall protection indicating the employees had not retained the understanding and skill necessary to work safely.
