29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):        (a)  Jobsite:  On or about June 24, 2014, four employee were installing an asphalt shingle roof and they were not protected from falling to t he ground.  The fall hazard to the four employees was about 22 feet to the ground.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate  understanding and skill required by paragraph (a) of this section:Note:  Circumstances where retraining is required include, but are not limited to, situations where (1) Changes in the workplace render previous training obsolete; or (2) Changes in the types of fall protection systems or equipment to be used render previous training obsolete; or (3) Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:       (a) Jobsite:  On or about June 24, 2014, the employer did not retrain his employees in the hazards of working from a roof and in how to minimize those hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       (a) Jobsite:  On or about June 24, 2014, the employees were using an aluminum extension ladder to access the roof and this ladder only extended above the edge of the roof by 2 1/2 feet.   The fall hazard to the employees was about 22 feet to the ground.
