29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):  a)  Milk Pasteurizing Room, on or about 6/26/14:  Platform at the top of fixed ladder for the UF Concentrate tank, six feet from floor level, was not provided with midrails.  b)  Whey Process, on or about 6/26/14:  Platform at the top of a fixed ladder for the Nano surge tank, 51 inches from floor level, was not provided with midrails.  c)  Whey Process, on or about 6/26/14:  Platform at the top of a fixed ladder for the cream balance tank, 51 inches from floor level, was not adequately guarded in that the single railing provided was only 26 inches high.   d)  Whey Process, Drying Room, on or about 6/26/14:  Access platform for the air valves, six feet from floor level, was not provided with midrails.  e)  Whey Process, on or about 6/26/14:  Two platforms at the top of fixed ladders for two permeate dryer cyclones, were not provided with midrails.
