29 CFR 1926.1053(b)(1): The ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  24 Maverick St., Rockland, Maine - Two employees were accessing the pump jack scaffold with a portable ladder that only extended 1.5 feet above the upper landing surface.
