29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A. Titan Excavating, LLC worksite located at 2092 Center Rd., Avon, Ohio: On or about June 20, 2014, the employer did not ensure an employee was protected from head injuries while working below and near the excavator being operated. Due to this condition, employees were potentially exposed to impact injuries caused from falling material and/or equipment.

Titan Excavating, LLC worksite located at 2092 Center Rd., Avon, Ohio: On or about June 20, 2014, the employer did not ensure an employee was adequately protected from cave-ins while installing a waterline in a near vertical trench that was approximately 6.00 feet deep.
