Instance: The employer allowed his employee to operate a Hitachi 2-1/2 pneumatic nail gun while not wearing any eye and or face protection exposing the employee to possible eye and face injuries from flying debris.   2nd. Instance: The employer allowed his employee to operate a Dewalt table saw to cut exterior molding while not wearing any eye and or face protection.     Conditions occurred on or about 06/26/14.
