OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to crushed-by hazards from two Rotary single pole vehicle lifts were not maintained in safe operating condition:   a) Automotive bays 1 and 2: An employee operates two Rotary single pole vehicle lifts to perform tire changes with both lifts missing their safety legs.

Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident.  a) 648 Johnson Ferry Road Managers work area: The maintenance manager could be exposed to potential shock hazards of 110 volts from two unmarked circuit breaker boxes that were open and only 2 feet from his work area.
