29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a.  An employee working as a trap personnel was exposed to a noise dose of 178.6%, or an equivalent dBA (decibels, A scale) of 94.1 dBA.  The sampling was performed on , for 420 minutes during one work shift on July 1, 2014. The employer did not administer a continuing and effective hearing conservation program as described in 29 CFR 1910.95(c) through (o):    1.The employer did not conduct noise monitoring at areas where exposure levels were suspected to exceed the 85 dBA;      2.Employees had not been sent for  a baseline audiogram and then annually thereafter;      3.Employees had not been trained at new hire and then annually thereafter on the effect of noise on hearing, the purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selections, fitting, use, and care; and the purpose of audiometric testing, and an explanation of the test procedures; and    4.The employer did not have available for the employees a copy of the hearing conservation standard.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:   a. On or about July 1, 2014, an employee working as trap personnel used an 3M8511 N95 disposable dust mask on a voluntary basis while mowing the grass at the shooting field and the employer had not provided the basic information contained on appendix D of the respiratory protection standard.
