29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high:  (a) 851 E. Columbus Ave., Springfield, MA: On or about June 27, 2014, there was no midrail for the guardrail system installed at the elevator shaft on the 2nd - 5th floors at this address.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:  (a) 851 E. Columbus Ave., Springfield, MA: On or about June 27, 2014, the top/only rail of the guardrail system installed on the 2nd-5th floor(s) would not withstand 200 pounds of force without failing, as it could have been pulled out of the wall with far less force than that.     Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
