29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Warehouse aisle five: An extension cord was used to power a power strip that serviced a Doran scale model 7000 xlm and a computer, on or about 6/30/14.  b) Warehouse aisle eight: An extension cord was used to power a scale, on or about 6/30/14.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer:    a) Warehouse and dispensing/receiving area: The employer did not train all employees on the new symbols located on chemical containers that reflect those used with the globabl harmonization system, on or about 6/30/14.     NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
