29 CFR 1926.502(d)(16)(iv): Personal fall arrest systems, when stopping a fall, did not bring an employee to a complete stop and/or and did not limit maximum deceleration distance to 3.5 feet (1.07 m):    a) An employee performing painting operations from a Nifty LWI-2507 telescopic boom aerial lift was attached to the basket with a lifeline and lanyard, each of which had a deceleration device, thereby exceeding the maximum deceleration distance of 3.5 feet.

29 CFR 1926.502(d)(19): Personal fall arrest systems and components subjected to impact loading were not immediately removed from service and/or were reused before they were inspected by a competent person and determined to be suitable for reuse:  a) An employee working in a Nifty LWI-2507 telescopic boom aerial lift was attached to the basket with a lifeline and lanyard, where the deceleration device on the lifeline was deployed and was not immediately removed from service.
