29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:       a)  Hybrid Remodeling Group LLC at 6394 E. 62nd Place, Commerce City, CO 80022:  On or before June 30, 2014, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:       a)  Hybrid Remodeling Group LLC at 6394 E. 62nd Place, Commerce City, CO 80022:  On or before June 30, 2014, the employer did not ensure that employees were using fall protection when shingling a residential ranch  roof with a 6 on 12 pitch.    Condition exposed employees to fall hazards of up to 10 feet 4 inches.
