29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     a) Employees were exposed to eye and face injury while operating an electric saw without the benefit of eye and face protection. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a) Employees were exposed to the hazard of falling approximately 16 feet where a portable ladder did not extend at least 3 feet above the landing surface of a second story deck.        In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):         a) Employees were exposed to the hazard of falling while performing carpentry work from a second story deck having a height of approximately 16 feet without the use of fall protection.        b) Employees were exposed to the hazard of falling while hoisting from a second story deck having a height of approximately 16 feet without the use of fall protection.        Wood Structures Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 190947, citation 1, item 1, and was affirmed as a final order on March 6, 2012, with respect to a worksite located at 210 Park Ave., Glenco, Illinois, 60022.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
