29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.)  Masters Mechanical, Corp., Fort Eustis, Building 664:  On or about 06/26/14, employees replacing an air handler for the HVAC unit on the roof were not protected from falling 26' to the ground.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Masters Mechanical Corp., Fort Eustis, Building 664:  On or about 06/26/14, employees were using a 12' ladder to access an upper landing surface that was not extended at least 3 feet above the landing surface.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Masters Mechanical Corp., Fort Eustis, Building 664:  On or about 06/26/14, employees were improperly using a 12' self supporting ladder to access the roof to work on the HVAC system, in that they were using the ladder as an extension ladder.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
