29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Meat Department - On or about 07/02/14 and at times prior thereto the Hobart cuber/tenderizer Model 403 was operated with a bypassed interlock system, without the guard in place. The point of operation (ingoing nip points created by the two cutting rollers) were not guarded.    b) Storage Area - On or about 07/02/14 and at times prior thereto the green baler (Birmingham Fabricating Co., Model PB 6030 S, S.N. 110 1173) was operated with a disabled interlock system that kept the door permanently in the open position. The point of operation was not guarded.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
