29 CFR 1910.23(b)(1): Every wall opening from which there is a drop of more than 4 feet was not guarded:     a)      Trans State Airlines, LLC @ 5260 Banshee Rd., Hanger 42, Hazelwood, MO 63042: On and before July 2, 2014, the employer did not ensure that employees were protected from a fall hazard greater than 4 feet. Mechanics entered the aft cargo compartment of an Embraer EMB-145 to conduct a line check of the area in accordance with standard operating procedure 5-20-45. The aft cargo compartment was assessable via a 39 and ¼ inch door opening at the rear of the plane. The door remained open while the mechanic performed the line check. This condition exposed the mechanic to a fall hazard of approximately 72 inches.

29 CFR 1910.26(c)(3)(v): The employer did not ensure that the employee faced the ladder when ascending and descending:     a) Trans State Airlines, LLC @ 5260 Banshee Rd., Hanger 42, Hazelwood, MO 63042: On and before July 2, 2014, the employer did not ensure that employees faced the ladder when they ascended and descended the Cotterman, five step, platform ladder. The Cotterman platform ladder was not of sufficient height to reach a desired work level. The platform height of the Cotterman platform ladder was approximately 51 inches. The desired working level height, inside the aft cargo compartment of an Embraer EMB-145, was approximately 72 inches. This condition caused the employee to climb over the platform ladder handrail.
