On July 2, 2014, at 5960 Sunbury Road, Westerville, Ohio, employees were working from an elevated scaffold platform 16 feet wide supported by a rough terrain forklift. Due to the platform width, the platform ends moved vertically several feet as the employees walked back and forth along the platform, exposing the employees to a 20 foot fall hazard.
