29 CFR 1926.416(a)(3): Employer did not advise employees of the location of energized electrical power circuits, the hazards involved, and the protective measures to be taken:    (a)  On or about June 30, 2014 at the pump controls in a penthouse to the General Mitchel International Airport, the employer did not advise employees of the location, energized state, the potential hazards associated with the 480 VAC present, and the protective measures to be taken when working on the equipment. This exposed an employee to electrical shock hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    (a) On or about June 30, 2014 at a pump control in the penthouse of the General Mitchel International Airport, an employee working on an 480 VAC pump, which was not de-energized down stream or guarded to prevent accidental contact with live parts, was shocked and injured.
