29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) Employees were exposed to potential eye injuries while using pneumatic nail guns, without appropriate eye protection, on a commercial building and the employees had not recieved instruction from the employer to recognize and avoid the unsafe conditions found at the job site.  b) An employee was exposed to struck-by hazards while performing clean-up activities without appropriate head protection, along side a commercial building while other employees worked on the roof above him and the employee had not received instruction from the employer to recognize and avoid the unsafe conditions found at the job site.
