29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. A energy control program (lockout/tagout) was not established according to the requirements of the standard including the use of locks (or comparable locking or lockable equipment) to isolate equipment, ensuring that more than one lock be used when more than one employee works on the same equipment at the same time with another employee, procedures to ensure equipment energy is isolated, training (documented) for all employees involved, and the other provisions of the standard, such as annual inspection and certification (note: the lockout program is required to be in writing with written specific procedures). Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
