29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): (a) Jobsite, 601 Welbourne Drive, Seven Fields, PA 16046: On or before July 7, 2014, employees were working from levels that were not fully decked. The employees were exposed to fall hazards between approximately 19 and 28 feet 6 inches while removing and installing siding.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) Jobsite, 601 Welbourne Drive, Seven Fields, PA 16046: On or before July 7, 2014, employees were working unprotected and exposed to fall hazards between approximately 19 and 28 feet 6 inches while removing and installing siding from scaffolding that was not equipped with guard railing.
