OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing type injuries from inadequately supported steel "Tee" beam that was being prepared for welding:    Worksite:  The employer did not ensure through providing specific training, instruction and/or visual observation that the employee, performing tack-welding tasks to install "T" steel beams onto the existing "I" beams of a structure was performing the tasks safely and appropriately at all times to prevent the "T" beams from falling or displacement.      On or about July 7, 2014, one of "T" beams displaced and fell soon after the forks used to support the beam lowered down due to insufficient welds done on one of ends of the beam. The employee was pinned in between the beam and a column and sustained serious injuries from the incident.
