29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about July 7, 2014, at the above addressed worksite, an employee was exposed to the hazard of falling a distance of 23'-0" to ground level while installing a gutter from a low-slope roof without fall protection.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:     a.  On or about July 7, 2014, at the above addressed worksite, an employee was exposed to a fall hazard while working from an extension ladder placed at a passageway without being secured or barricaded.
